I call Optum to have my health records faxed to my new location. they answer the phone and say to use their Website to fill out the patient records release form.

Their Website has a download function for a patient records release form, however it a broken link. The Webpage says New Mexico, oh I have to go to New Mexico to download the form? This is ridiculous!

I try again to log into the Optum Patients Portal from the state of my last residency, the portal doesn’t even come up, after 5 minutes wait time, I decided this is another broken link.

I call Optum again, the lady called Karen was listening and helping me with my request, thanks this is how it should be.
